Please complete a consent form before your next service. If you haven’t submitted one in a while, a new consent form will be required starting January 1, 2025. Once completed, you will not need to fill out another unless there are updates or changes in your health or treatments. In that case, please complete the Health Update Waiver and Consent Form.
